As the Care Team Manager for Grand Rapids, you will be responsible for leading the daily operations of the Care Team, including Registered Nurses (RNs), Nurse Practitioners (NPs), LICSWs, Health Partners, and other staff. You will manage team members, oversee schedules, develop and enforce policies and procedures, and ensure smooth care delivery both virtually and in our patient's homes. As a key leader within the Michigan market, you will be responsible for building and developing a high performance team, establishing and meeting success metrics for the region, managing relationships with local providers and other key partners, and partnering with the Market Operations Leader and Medical Director on strategic initiatives.
The expected compensation for this role is $120,000 to $150,000 annually, depending on location & experience. This will be an in-office role with regular hours Mon - Fri from 8:30 AM to 5 PM.
