Job summary

Daymark Health Inc. in Lansing seeks a Care Team Manager to lead the daily operations of the Care Team, including RNs, NPs, LICSWs, and other staff, delivering care virtually and at patients' homes.

You will build a high-performing team, establish regional success metrics, and partner with local providers and the Medical Director on strategic initiatives to grow Daymark's Michigan footprint.

Qualifications

• 5+ years of experience in healthcare operations, care management, or related clinical leadership.
• Experience managing people and building high-performing teams.
• Strong staffing, scheduling, and budget management skills.
• Data-driven mindset with ability to monitor clinical outcomes and drive improvement.
• Excellent interpersonal and communication skills with patients, families, and partners.

Responsibilities

• Lead day-to-day clinical operations, including staffing, scheduling, and adherence to SOPs.
• Build and sustain a high-performing team culture delivering outstanding patient outcomes and experience.
• Hire, onboard, coach, and develop clinical and administrative staff.
• Partner with Market Operations Leader and Medical Director to support market growth and alignment.
• Use data to monitor performance, identify opportunities, and drive timely action.
• Set metrics for quality, patient experience, productivity, and efficiency, and hold the team accountable.

RN/NP/LICSW licensure
