Job Title: Care Support Coordinator

Location:Pittsburgh, PA

Schedule:Full-time, Hybrid, Monday–Friday. Flexible schedule required, including evenings and weekends to accommodate family needs and court orders.

About the Team

The Care Support Coordinator position is part of our Family Support Services team. This team provides a wide array of family support services to help families and children grow stronger, from prevention and reunification services to care coordination services. All of our programs are built around the idea of permanency — supporting safe, nurturing, and lasting families — and putting the health and safety of the child first. This position independently develops and implements service plans that promote safety, well-being, and permanency for all assigned clients.

About the Position

TheCare Support Coordinatorprovides direct, community-based services to families and children with the goal of promoting safety, stability, and well-being. This role involves assessing family needs, developing individualized service plans, supporting parenting skill development, supervising family visitation, and connecting families with appropriate community resources. The Care Support Coordinator is expected to maintain accurate documentation, adhere to regulatory standards, and work collaboratively with county and community stakeholders to support family preservation and reunification.Job responsibilities include:

  • Assess client and family needs using formal and informal evaluations, referral documentation, and family input.
  • Develop and implement individualized service plans focused on safety, stability, and parenting capacity.
  • Complete all required documentation (e.g., service plans, progress notes, contact logs) accurately and on time using the electronic health record system.
  • Track and meet required billable service delivery targets.
  • Maintain professional boundaries and confidentiality; model ethical conduct and culturally responsive behaviors in all interactions with families, colleagues, and partners, consistent with agency values.
Qualifications
  • Bachelor’s degree in social work or relevant Human Service field required
  • Minimum of one year experience working in Child Welfare
  • No prior experience required with a qualifying Human Services degree
  • Minimum of 1 year of experience required with a related (non-Human Services) degree
  • Experience working with diverse populations and/or individuals with disabilities preferred
  • Act 33/34 Clearances
  • FBI Clearance
  • Valid, current driver’s license and personal automobile liability insurance with a minimum of $300,000 combined single limit coverage
  • Approval to verify driving history at the time of authorization and annually; approval to verify insurance coverage at the time of authorization and semi-annually
  • Reliable automobile with current inspection
  • Ability to meet physical and mental requirements of the job
