Job summary

Westborough Healthcare is seeking a licensed social worker (MSW, LICSW) to lead resident care and coordinate services in a long-term care setting. You will support residents and families, collaborate with the care team, and guide paperwork and applications for financial assistance.

The role requires a Master’s in Social Work or related field, a valid Social Work license, and 1–3 years of experience in long-term care. Strong communication and empathy are essential.

Qualifications

  • Master's degree in Social Work, Psychology or a related field is required.
  • Social Work License is required.
  • Minimum 1–3 years of long-term care experience preferred.

Responsibilities

  • Promote positive morale and provide counseling to residents and families.
  • Lead the interdisciplinary team with administrators and medical staff to plan and coordinate care.
  • Assist residents with applications and qualification for financial assistance.
  • Review Resident Rights with new admissions and ensure admission paperwork is complete.
  • Schedule and coordinate care planning meetings and write progress reports.
  • Coordinate discharge planning and arrange follow-up care as needed.
