Job summary

Oakmont Management Group is seeking a Care Provider at Ivy Park at Wood Ranch in Simi Valley, CA. You will assist residents with daily living activities, respond to needs promptly, and communicate changes to supervisors. The role emphasizes safety, dignity, and teamwork within a supportive senior living community.

The position requires a genuine care for the elderly, HS diploma or GED, and willingness to participate in health screenings and background checks as part of our safety standards.

Responsibilities

• Assist residents in Activities of Daily Living per care plan (eating, bathing, dressing, toileting, transferring, continence).
• Respond promptly to residents’ needs while maintaining dignity and safety.
• Communicate any changes in condition to a supervisor immediately.
• Maintain a safe, secure environment following safety standards.
• Encourage teamwork with coworkers and other departments.

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents’ independence and quality of life, in accordance with the company’s mission and values.

Responsibilities:

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident’s service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

Qualifications:

• High School Diploma or GED
• Strong desire to work with the elderly and care for their needs
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
