Job summary

Vanderbilt Health in Lebanon, TN seeks a Care Partner to support nursing staff and patients with basic care needs, execute orders, and help evaluate care within the unit. Reports to the charge nurse and Clinical Director, collaborating with clinicians, patients, and families to ensure quality care.

Qualifications

• Interpersonal communication skills necessary for care coordination.
• Ability to be self-directed and quality focused in a clinical setting.

Responsibilities

• Obtain vital signs, intake and outputs, and daily weights.
• Report abnormal findings to nurse/charge nurse.
• Assist patients with activities of daily living (ADLs).
• Transcribe physician orders and relay messages as needed.
• Assist physicians with obtaining patient information when required.
• Follow infection control standards and patient safety practices.
• Support care team with documentation and care planning.
