We are working with a vibrant and friendly GP Practice in West Yorkshire looking for an experienced Care Navigator to join their team. This is a high-performing surgery with a great retention rate. Due to the demand and their plans for team expansion, a new position has become available for a Care Navigator.

Based in South west Bradford, this GP practice has 25 hours available, worked over 5 days per week. and if successful, you will join a surgery with a good reputation and a patient size of just over 10,000.

The ideal candidate will have previous general practice experience, familiar with SytmOne, and the ability to assign patient requests to the appropriate clinicians.
