Job summary

UnitedHealth Group is seeking a Telehealth FMC program agent for a National Remote role in the United States. The position conducts telephonic outreach to recently discharged members, assists with transportation, medication, and PCP follow-up, and documents conversations in Pathway.

Must have 2+ years in clinical or outpatient settings and be able to work 8-hour shifts during Pacific business hours with occasional overtime.

Qualifications

  • Minimum education required: high school diploma or GED or equivalent.
  • Experience in telecommunication with patients/matients preferred.
  • Must be 18+ and able to work full-time.
  • Clerical/administrative skills and ability to document conversations clearly.

Responsibilities

  • Guiding members on utilization of plan benefits and follow-up with PCP
  • Documenting member conversations in Pathway and ensuring accurate FMC assessment
  • Transferring members to Member Services for follow-up
  • Providing coverage to colleagues on PTO as needed
  • Adhering to telecommuter policy and security requirements

The Telehealth FMC program is a non-clinical- non, non-licensed healthcare resource that conducts telephonic outreach to members who have recently discharged from the hospital. The agent provides resource assistance to members providing information regarding transportation, medication and primary care follow-up services. This assists the member in ensuring timely follow-up with Primacy care providers (PCPs), transportation services to appointments and medication follow-up.

This position is full time Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7:30 am - 5:00pm PST. It may be necessary, given the business need, to work occasional overtime.

We offer 1 week of paid training. The hours during training will be 8:30am to 5:00pm CST, Monday - Friday after they will move to the on-the-job training program during normal work schedule.

Primary Responsibilities:
  • Guiding members on utilization of plan benefits such as follow-up with PCP, transportation and medication follow-up.
  • Completing the FMC assessment in Pathway and accurately documenting member conversations.
  • Transferring members to member services when needed for follow-up.
  • Other duties as assigned.
  • Provide coverage to colleagues when on PTO as needed.

Required Qualifications:
  • High School Diploma / GED OR equivalent work experience
  • Must be 18 years of age OR older
  • 2+ years experience working in a physicians office, Skilled Nursing Facility (SNF), Assisted Living Facility (ALF), or outpatient clinic with medical team communication and collaboration
  • 2+ years of experience with handling inbound and/or outbound calls with patients / members
  • Ability to work an 8-hour schedule during the work hours of 7:30am-5:00pm Pacific, Monday-Friday
Preferred Qualifications:
  • Experience in Microsoft Word (edit, create, and save documents), Microsoft Excel (adding, sorting and filtering data) and Microsoft Office (email, folders, attachments, and calendar)
  • Administrative and organizations skills. Ability to self-direct work, multitask multiple deliverables
Telecommuting Requirements:
  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Soft Skills:
  • Strong telephonic and written communication skills

The salary for this role will range from $20 - $36 hourly based on full-time employment. We comply with all minimum wage laws as applicable.
