Job summary

Providence Kadlec Regional Medical Center is seeking a Team Based Care RN to independently apply the nursing process for a panel of patients, with opportunities to share visits with providers and conduct independent visits under standardized protocols.

You will collaborate with the care team to coordinate and execute the plan of care, demonstrating initiative in a team-based care model while delivering safe, compassionate patient care in a fast-paced environment.

Qualifications

• Graduate from an accredited school of Nursing.
• Upon hire: Washington Registered Nurse License
• 2 years Professional nursing experience.

Under the general direction of the Supervisor/Manager of Clinic Operations with clinical oversight from the Director of Clinical Operations, RN: The Team Based Care (TBC) Care Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and evidence-based workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model.

Required Qualifications:
