Job summary

Memorial Home Services in Taylorville, IL is seeking a part-time Care Manager to coordinate utilization review, discharge planning and case management. You will mobilize internal and external resources to optimize patient value and reimbursement.

As a key member of the patient care team, you will evaluate patient utilization, ensure appropriate stays, and contribute data for quality improvement. The role requires an Illinois RN license with at least two years of acute care experience.

Qualifications

• RN licensed in Illinois is required; BSN preferred.
• Minimum of 2 years acute care nursing experience.

The Case Manager is an active member of the patient care team, providing clinical expertise, problem identification and data collection for quality improvement initiatives.
