Job summary

Abound Health NJ is seeking a Care Manager to join our team in New Jersey. You will report to the Program Director and provide support, advocacy, and guidance to individuals receiving New Jersey Waiver Services while upholding the organization’s mission and values.

Qualifications

• Master’s degree plus 1 year direct work with persons with disabilities or equivalent
• Bachelor’s degree plus 2 years' experience
• Associate’s degree/60 credits plus 4 years’ experience or higher
• High school diploma/GED with 5+ years’ experience

Responsibilities

• Review client service/grid notes to ensure regulatory-compliant documentation
• Maintain accurate client and staff records
• Conduct clinical and supervisory meetings, annual evaluations, and staff development plans
• Participate in staff and ISP meetings; advocate for client needs
• Develop individualized goals, task analyses, and interventions
• Complete assessments, progress reports, and AENs as required
• Oversee staffing patterns, scheduling, and PTO/payroll requests
• Ensure client trainings and OnTarget goal entry are accurate
• Resolve grievances and complete incident reports within timelines
• Build community relationships and represent Abound Health NJ at events

Abound Health NJ is seeking a Care Manager to join our dedicated team in New Jersey. In this role, you will report to the Program Director. The Care Manager is responsible for providing support, services, advocacy, and guidance to individuals receiving New Jersey Waiver Services while upholding Abound Health NJ’s mission, vision, and core values. You will ensure high-quality service delivery, maintain compliance, and support both clients and direct care staff. This is a remote role for applicants who live in NJ.

Key Responsibilities

• Review client service/grid notes and ensure documentation meets regulatory standards
• Maintain accurate and up-to‑date client and staff records
• Conduct clinical and supervisory meetings, annual evaluations, and staff development plans
• Participate in staff meetings and client ISP meetings; advocate for client needs
• Develop individualized goals, task analyses, and interventions
• Complete assessments, progress reports, and AENs as required
• Oversee staffing patterns, scheduling, and PTO/payroll requests
• Ensure completion of client-specific trainings and accurate OnTarget goal entry
• Resolve client/guardian grievances and complete incident reports within required timelines
• Build community relationships, represent Abound Health NJ at events, and support resource access

Qualifications

• Ability to manage changing priorities and work in a fast-paced environment
• Excellent communication and interpersonal skills with clients, families, staff, and community partners
• Ability to multitask, problem-solve, and maintain professionalism under pressure
• Strong understanding of ISP implementation, goal writing, task analysis, and Medicaid/third-party billing
• Ability to work collaboratively within a team and maintain therapeutic relationships
• Strong attention to detail, accuracy, and organization

Education

Candidate must meet one of the following credentialing requirements:

• Master’s degree + 1 year of experience working directly with persons with disabilities
• Bachelor’s degree + 2 years of experience
• Associate’s degree or 60 credits + 4 years of experience
• High school diploma/GED + 5+ years of experience

Pay Range: $50,000–$55,000 annually

Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. Total compensation may include additional forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits beginning on your 60th day of employment. Our 401(k) program is available after 12 months of employment.
