Job summary

Providence Regional Medical Center Everett is hiring a Care Manager MSW & LCSW for per diem roles in acute care. The Inpatient Care Manager provides patient-centered care management, including assessment, planning, coordination, discharge planning, and advocacy to address social and financial barriers.

Two track positions: MSW and LCSW, require education and appropriate licenses; experience in healthcare and acute care preferred, with TIPs/Case Management training.

Qualifications

• Master's Degree in Social Work required from CSWE-accredited program.
• Washington Clinical Independent Social Worker License required upon hire.
• Minimum 2 years social work experience in healthcare; 1 year in acute care or TIPs/Case Management Orientation training.
• Preferred ACM/CCM certification and prior case management experience.

Responsibilities

• Assess and plan for at risk patients within acute care.
• Coordinate care and discharge planning.
• Document interventions and ensure regulatory compliance.

Care Manager LCSW

The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management LCSW is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral or legal barriers to accessing necessary resources and care.

Required Qualifications:

Master's Degree in Social Work from an Accredited Council on Social Work Education program

Upon hire: Washington Clinical Independent Social Worker License

2 years SW experience in any healthcare setting

1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program

The Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management MSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access.

Required Qualifications:

Master's Degree in social work from an accredited CSWE program

Upon hire: Washington Clinical Independent Social Worker Associate License

1 year SW experience in any healthcare setting

1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
