Job summary

Physicians Regional Medical Center - CAMPUS is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join the Care Management team. The Care Manager LVN/LPN coordinates patient care, discharge planning, and utilization review under supervision of a RN or physician.

The role emphasizes review of medical records, collaboration with interdisciplinary teams, and ensuring safe transitions of care in compliance with standards.

Qualifications

• 1-2 years of clinical acute care nursing experience required.
• Experience in utilization review, case management, or discharge planning preferred.

Responsibilities

• Conduct daily reviews of medical records for appropriateness and medical necessity and escalate as needed.
• Identify and document avoidable hospital days and communicate findings to UM committee.
• Coordinate discharge planning including arranging post-hospital placements and ensure continuity of care.
• Maintain relationships with community agencies to facilitate referrals and transitions.
• Assist in interdisciplinary meetings and support collaborative care planning.
• Ensure accurate and timely documentation of utilization review and discharge planning activities.
• Direct others to comply with federal reporting mandates for first-person reporting.
• Participate in developing and evaluating utilization management and discharge planning programs.
