Job summary

Jobtailor is seeking a licensed practical nurse to perform in-home or facility assessments and coordinate care in community settings across Mississippi. You will work with care managers and interdisciplinary teams to determine level-of-care, develop care paths, and plan discharges based on client needs.

You will document assessments using GDS, MMSE, and fall risk tools, ensure timely follow-ups, and maintain confidential records while adhering to Medicare/Medicaid regulations.

Qualifications

• Must be a graduate of an accredited LPN program.
• Current Georgia licensure and CPR certification.
• Minimum 2 years of nursing experience in community health, long-term care, or geriatrics.
• Understanding of Medicare/Medicaid regulations and care coordination best practices.
• Ability to function independently in a community-based setting.
• Strong clinical documentation and time-management skills.
• Familiar with EMR systems and standard office software.
• Willingness to travel to client homes.

Responsibilities

• Complete and document assessment tools, including GDS, MMSE, and fall risk assessments
• Schedule and perform timely assessments, reassessments, and follow-up per DCH policy
• Coordinate transitions from hospital to home/community settings
• Document care plans, hospitalizations, care transitions, services, and case notes per DCH and VNHS standards
• Approve services within established funding limits and ensure clinical appropriateness
• Participate in interdisciplinary team meetings, training sessions, and quarterly network events
• Support infection control practices and maintain confidential client records
