ECU Health in Washington, NC is seeking a Care Manager to coordinate the plan of care with the provider, interdisciplinary team, patient and family. This full‑time role focuses on annual wellness visits and works Mon–Fri 8:00am–5:00pm, with no nights, weekends, or holidays.

You will manage a designated caseload, promote efficient use of resources, and drive performance improvements in workflow and patient satisfaction.

Qualificações

• RN licensure required by NC Board of Nursing.
• 0–3 years of Medical Case Management experience.
• Case management and ambulatory experience preferred.

Position Summary

• The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.
• The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction).
• The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well‑organized and safe plan of care is established for every patient.

Minimum Requirements

• Education: Associate Degree in Nursing or higher is required.
• Current licensure as a Registered Nurse by the NC Board of Nursing is required.
• Experience: 0-3 years of Medical Case Management
• Case management and ambulatory experience is preferred

Pay Range

• Care Manager I salary range > $65520.00 - $95501.12
• Final compensation is determined at the offer stage.

Other Information

• Location: ECU Health Internal Medicine (outpatient) - Washington, NC
• This position is full‑time (40hrs/week) - Monday to Friday, 8:00am - 5:00pm
• No nights, weekends, call or holidays
• This position will focus on conducting annual wellness visits.
