Job summary

Southwest Michigan Behavioral Health (SWMBH) in Portage, MI is hiring a Care Management Specialist II to help Medicaid members access appropriate mental health and substance use services. You will perform utilization review, care coordination, and brief/crisis counseling within the Integrated Care Team.

Requirements include a Master’s degree in a related field, active license (LMSW/LP/LPC/RN), and experience in behavioral health.

Qualifications

• Master’s degree in Social Work, Psychology, or related Human Services field.
• Active LMSW/LLP/LPC or RN license required.
• Experience in mental health and/or substance use services.
• Knowledge of DSM and levels of care (inpatient, partial hospital, IOP, outpatient).
• Strong phone, communication, and organizational skills; proficient with Microsoft Office.

Responsibilities

• Complete Level II assessments (ASAM and LOCUS) and build care plans with members and the Integrated Care Team.
• Screen referrals, determine level of care, and make medical necessity decisions.
• Coordinate care across providers and manage transitions between hospital, sub-acute, and community settings.
• Support urgent and crisis care, including ED and inpatient diversion.
• Provide brief counseling and SBIRT-based interventions; connect members to resources and referrals.
• Coordinate peer support services and collaborate with CMHs, health plans, and providers.

About the Care Management Specialist II Role

SWMBH manages behavioral health services for Medicaid members across eight counties in Southwest Michigan, under contract with the Michigan Department of Health and Human Services (MDHHS).

As a Care Management Specialist II, you’ll help members get the right mental health and substance use care at the right time through utilization review, care coordination, and brief/crisis counseling.

What You’ll Do as a Care Management Specialist II

• Complete Level II assessments (ASAM and LOCUS) and build care plans with members and the Integrated Care Team.
• Screen referrals, determine the appropriate level of care, and make medical necessity decisions.
• Coordinate care across providers and manage transitions between hospital, sub-acute, and community settings.
• Support urgent and crisis care, including emergency department and inpatient diversion.
• Provide brief counseling and intervention using the SBIRT model, and connect members to resources and referrals.
• Coordinate peer support services and collaborate with CMHSPs, health plans, and community providers.
• Document reviews in SWMBH systems, flag quality concerns, and recommend process improvements.

Care Management Specialist II Requirements

• Master’s degree in Social Work, Psychology, or a related Human Services field.
• Active LMSW, LLP, LPC, or RN license.
• Experience in mental health and/or substance use services.
• Working knowledge of the DSM and levels of care (inpatient, partial hospital, IOP, outpatient).
• Strong phone, communication, and organizational skills, and comfort with Microsoft Office.

Preferred

• Certified Addiction Counselor.
• Managed care or utilization review experience.
• Lived experience with behavioral health issues, or bilingual skills.
