Job summary

MaineHealth Behavioral Health is seeking a Care Management Social Worker I to assess biopsychosocial-spiritual status and collaborate with the care team on discharge planning. You will work with patients and families to determine options based on insurance, supports, and resources in context of their location and eligibility.

Required: MSW from an accredited school and Maine LMSW or LMSW-CC license; hospital experience is preferred.

Qualifications

• MSW degree from an accredited school.
• LMSW or LMSW-CC license issued by the State of Maine.
• Two years hospital experience preferred.

Responsibilities

• Conduct ongoing biopsychosocial-spiritual assessments of patients and families.
• Collaborate with interdisciplinary team to discuss discharge options and placement.
• Navigate insurance coverage, community resources, and eligibility factors for care transitions.
