Job summary

Humana Inc. is seeking a Manager, Care Management to lead a multidisciplinary team delivering Medicaid care management for Illinois members. You will guide care planning, coordinate resources, and ensure compliant, high-quality service delivery across the care management continuum.

Travel 50-75% and remote work with occasional onsite Humana Illinois meetings are expected. 40-hour work weeks with a competitive compensation range and bonus eligibility.

Qualifications

• Must reside in Illinois.
• Associate's Degree minimum.
• Active RN or SW license.
• 5+ years professional experience.
• 2+ years management/supervisory experience.
• Data analysis and trend interpretation.
• Strong communication and teamwork skills.
• Knowledge of Medicaid/Medicare and long-term care policies.
• Proficient with MIS software and Microsoft tools.

Responsibilities

• Oversee assessment and evaluation of members' needs to achieve optimal wellness.
• Guide members and families toward appropriate resources and care planning.
• Collaborate with internal teams and community partners to integrate non-clinical support.
• Monitor program performance and drive continuous improvement.
• Ensure compliance with federal, state, and contractual requirements.
• Develop policies and procedures for consistent service delivery.
• Lead care management extenders like CHWs and peer specialists.
