Blue Shield of California is seeking a Senior Care Management – Nurse to oversee a DSNP caseload, review HRAs, and develop individualized care plans. You will engage members to reduce hospital readmissions and support the care management team. The role combines clinical leadership with program design in a hybrid work environment.
The role requires an active CA RN license, an ADN (BSN preferred), and 5+ years in nursing or healthcare, with managed care experience preferred. CCM is a plus.
The Care Management team coordinates, educates, and advocates care for the Dual‑Special Needs (DSNP) population with Blue Shield of California. The Case Management – Nurse, Senior will report to the Manager of Care Management within Medical Care Solutions. In this role you will be responsible for managing a caseload of DSNP members, reviewing Health Risk Assessments and completing Individualized Care Plans, engaging members to reduce readmissions to the hospital, and supporting the DSNP care management team.

Pay Range for California : $90860.00 to $136290.00
Pay Range for Bay Area : $102424.00 to $153636.00
