Job summary

Providence St. Joseph Hospital - Orange in Orange, CA is seeking a Care Management Coordinator for a full-time Day shift.

This clerical role supports the Care Management Department under licensed staff, coordinating communication among patients, providers, insurers, and other stakeholders to ensure safe transitions of care and accurate documentation. The successful candidate will have strong organizational skills, attention to detail, and the ability to manage multiple tasks.

Qualifications

• Minimum 2 years administrative experience, preferably in healthcare.
• Familiarity with healthcare terminology is a plus.

Responsibilities

• Coordinate communication between patients, providers, insurers and other stakeholders to ensure safe transitions of care.
• Provide clerical support for the Care Management Department and assist with documentation.
• Maintain accurate records and support delivery of quality patient care.
