Qualifications

  • Master’s degree in Counseling, Social Work, or Marriage and Family Therapy is required.
  • Must hold/maintain Ohio licensure, independent licensure preferred.

Responsibilities

  • Assess patients' mental health needs and develop individualized care plans.
  • Coordinate care with providers and interdisciplinary team to ensure continuity.
  • Educate patients and families about conditions and treatment options.
  • Perform brief therapies and counseling.

Clinical knowledge and skill used through advocacy, providing recommendations, and care coordination. Works closely with leadership, internal partners, external agencies, and other youth systems to reach shared goals and implement treatment recommendations. Provides ongoing support with patient and family before, during, and after acute services.****Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.****Job Description Summary:**Provides case management procedures, therapeutic interventions and clinical care coordinationfor children, adolescents and their families.**Job Description:****Essential Functions:*** Conducts assessments of patients' mental health needs and develops individualized care plans.
