GT Independence is hiring a Care Integration Senior Aide (CISA) to conduct in-home visits and work with Direct Support Professionals within the Community Supported Living program. You will observe, document, and report changes in clients’ health and social conditions and support quality service delivery.

The Care Integration Senior Aide (CISA) conducts in-home visits to work directly with clients and Direct Support Professionals (DSPs). The CISA observes, documents, and reports changes in clients’ health and social conditions, collaborates with the care team to help prevent avoidable negative outcomes, and supports quality service delivery throughout the Community Supported Living (CSL) program.
Overall Responsibility:
Provide advanced, non‑clinical support to clients and Direct Support Professionals (DSPs) within the CSL program. The CISA focuses on observing, documenting, and communicating changes in clients’ health or social conditions and supporting quality service delivery through coaching and coordination.
While the CISA supports DSPs in the field, this role does not include direct supervisory authority and does not perform scheduling, disciplinary, or performance management functions.
