ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to support adolescents and families. Under the supervision of the Program Director, you will coordinate orientation, transportation, doctor visits, and scheduling to ensure seamless, high-standard care.

You will supervise clients, facilitate groups, conduct admissions orientations with assessments, and maintain EMR documentation. Strong boundaries and teamwork are essential, with a focus on confidentiality and timely communication.

Qualifications

• Minimum high school diploma; Bachelor’s degree preferred.
• Minimum of 2 years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.

Responsibilities

• Coordinate individualized care including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services meet high standards of care.
• Assist with client supervision and attendance at designated groups.
• Facilitate groups and activities as needed by the Facility Manager.
• Conduct client orientation on admission day and perform or assign assessments.
• Maintain compliance with standards and document shift reports in EMR.
• Coordinate transportation schedule and drive clients when needed.
• Document all transportation and group activity in EMR.
• Submit and document required documentation in EMR within 24 hours.
• Maintain confidentiality and professional boundaries.

Position Summary:
Under the supervision of the Program Director the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.
Duties and Responsibilities:

• Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
• Responsible for client supervision and ensuring they attend designated groups throughout programming.
• Facilitate assigned groups and activities as designated by the Facility Manager.
• Conduct client orientation for all admits on day of admission or assign to another staff if not available. Including UA screening, PHQ9 and other assessments as directed.
• Ensure compliance with Joint Commission Standards.
• Document shift reports and share pertinent information with the team.
• Coordinate the transportation schedule for all clients and drive clients as needed.
• Document all transportation and group activity in EMR.
• Administer and submit documentation in EMR for client Urine Analysis.
• Assist with facility cleanliness as needed.
• Responsible for coordinating client lunch daily.
• Serve as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude.
• Maintain the confidential nature of clients and related activities.
• Comply with the dress code.
• Complete tasks in a timely and accurate manner.
• Attend designated staff and in-service meetings.
• Any additional duties as assigned by the management team.
• Ability to maintain clear boundaries with clients and staff, set limits and manage clients when in need of direct supervision. Maintains appropriate and professional interactions with clients and staff.
• All documentation in EMR must be submitted within 24 hours.

Basic Education and Experience Required:

• The Care Coordinator will have a minimum of a high school diploma, Bachelor’s degree preferred.
• Minimum of two (2) years driving experience required.
• Minimum of 1-year working with kids in a therapeutic setting
• Experience with junior high or high school students is preferred.
• Must be able to communicate with people from all educational and cultural backgrounds.
• Must cope constructively with stressful situations.
• Must demonstrate tact, initiative, resourcefulness, professional manner, enthusiasm, and non-judgmental attitude.
