Harbor is seeking a Care Coordinator responsible for comprehensive, multi-system care coordination activities for youth with complex needs and their families.
Position is full-time, 40 hours per week, preferably based in Ottawa or Sandusky counties. The role is remote and community-based.
Minimum Qualifications
- At least 3 years of experience (or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree) in community-based services supporting children, youth, and families in areas such as behavioral health, child welfare, disabilities, juvenile justice, or related fields.
- Experience in family systems, community resources, case management, counseling, child protection, or child development.
- Culturally competent with training for managing complex cases.
- Ability to work with children and families experiencing SED, trauma, co-occurring disorders, and engaged with multiple child-service systems.
- Completion of high-fidelity wraparound training by a recognized entity (COE).
- Excellent verbal and written communication skills; ability to represent Harbor professionally.
- Proficiency in computer use.
- Valid driver’s license, acceptable driving record, and insurability.
- CPR/First Aid and NCI/CPI certifications within 90 days of hire.
- Preferably reside in the community served.
- Minimum internet requirements: 5 Mbps upload, 10 Mbps download, cable or fiber connection, Ethernet preferred.
Primary Responsibilities
- Engage children/youth with behavioral health needs and their families (caseloads: 1:10 for ICC, 1:25 for MCC).
- Maintain knowledge of community resources and link youth and families to supports.
- Conduct comprehensive assessments (CANS) and develop individualized, strength-based care plans.
- Monitor and update care plans, address challenges, and ensure services align with plans.
- Facilitate Child and Family Team meetings, ensuring family voice and choices, promoting community and natural supports.
- Coordinate access to clinical and psychiatric consultation.
- Develop and review safety and crisis plans with youth and families.
- Complete required documentation and contacts per CME rules.
- Work flexible hours, including evenings and weekends, as needed.
- Provide services in-home and in community settings.
