Position Overview: We are seeking a full-time Care Coordinator (Medical Assistant or LPN) to join our team in Haddon Heights, NJ. This role is ideal for a compassionate, organized, and detail-oriented healthcare professional who thrives in a dynamic and patient-focused environment. The Care Coordinator will be a crucial member of the care team, which includes a Medical Director and Nurse Practitioner, working collaboratively to ensure that patients within the senior living facility receive timely, comprehensive, and personalized care.

In this role, you will act as a central point of communication between patients, families, and healthcare providers, coordinating treatment plans and ensuring that patient needs are addressed promptly. You will manage clinical documentation, keeping patient records current and reflective of their ongoing needs and treatment. As part of the care team, you will oversee medication management, schedule tests, and coordinate specialist referrals to ensure seamless care for the patients.
