The goals of the Care Management Team include the achievement of optimal health, access to care and appropriate utilization of resources balanced with the patient's right to self-determination.

Qualifications

• Masters in Social Work from an accredited school of Social Work.
• Minimum of 3 years of Medical Social Work experience. Acute Care experience preferred.
• Maintains current license as Masters in Social Work in the State of CT (LMSW). Case Management certification ACM or CCM Strongly preferred.
• Ability to effectively utilize a variety of Microsoft Office, Outlook and Epic software, Morrissey and Web based programs required.
