Centene is seeking a remote-enabled role to support care management activities across the United States. You will engage members by phone or home visits, discuss care plans, and document services to ensure continuity of care and satisfaction.
The position supports coordination with providers and may include service assessments and maintaining records, with a focus on regulatory compliance and delivering high-quality benefits education.
Supports care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Interacts with members by performing member outreach telephonically or through home-visits and documents the plan for care/services of activities.
Applicants for this role have the flexibility to work remotely from their home anywhere in the United States. The work schedule is Monday - Friday, 8am - 5pm Central. Candidates with previous assessment experience and/or call center experience are preferred.
Requires a High School diploma or GED. Requires 1 - 2 years of related experience
Pay Range: $17.84 - $28.02 per hour

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law, including full-time or part-time status. Total compensation may also include additional forms of incentives. Benefits may be subject to program eligibility.
