Job description

Job Summary

The Care Coordinator I is responsible for providing care coordination services for clients in their assigned group. The coordinator will assess clients with all care management needs and address potential barriers to care to ensure timely diagnosis and treatment. Under the direction of the supervisor, the coordinator will maintain a clinical tracking system to ensure appropriate follow‑up, reporting, and billing.

Duties and Responsibilities
  • Process referrals (specialty care and ancillary services), verify insurances, and acquire authorization for services as needed.
  • Follow up on referrals, retrieve progress notes from specialists, close out the referral loop, and assist with obtaining second referrals if necessary.
  • Document activities performed on clients in the EHR and C3 database; update patient accounts and encounters using fields such as name, birth date, SSN, and account number.
  • Maintain PHI log in the EHR when referrals are sent or processed.
  • Update patient demographics in EPM, EHR, and C3 database.
  • Assist providers with follow‑up on referrals, no‑shows, and second referrals as directed.
  • Respond to phone calls from contractors and/or patients needing referrals or medical records.
  • Schedule, reschedule, and cancel patient appointments per established protocols for medical, obstetrics, dental, and behavioral health services.
  • Fax patient records to internal EHR and acquire provider signatures for faxes requiring approval.
  • Flag charts for staff alerts regarding non‑compliance services or demographic updates.
  • Assist clinical teams by reviewing reports or charts for abnormal labs/findings and notifying clinicians in emergencies.
  • Demonstrate strong customer‑service skills and proper phone etiquette when communicating with patients by phone, text, or patient portal.
  • Complete daily tasks assigned by the Director or Supervisor and run rescreen reports to bring clients back into service.
  • Attend weekly department meetings.
  • Uphold CentroMed’s mission, values, ethics, integrity, safety, and corporate responsibility.
  • Maintain high levels of excellent customer service internally and externally.
  • Comply with all policies and procedures set forth by CentroMed.
  • Protect and secure all personal, confidential, and identifiable information in accordance with HIPAA and other confidentiality requirements.
  • Train new employees on job duties as needed.
  • Perform all duties in conformance with safety and security standards.
  • Perform other duties assigned as necessary.
  • Work on site as designated.
Qualifications
  • High School diploma or equivalent.
  • Medical Assistant certification preferred.

  • Computer proficiency and use of calculator.
  • Excellent mental ability for client interviews with attention to detail.
