Daybreak seeks a Care Coordinator to provide individualized, therapeutic support to YYAs experiencing homelessness, guiding them through housing placement and access to services that promote stability and well‑being.

You will build trust with clients, coordinate with providers, and help develop personalized treatment plans while maintaining trauma‑informed, culturally competent practice across the full continuum of care.

Qualifications

  • Associate’s degree or 2 years of college from an accredited program is required.
  • Two years of program management, case management, behavioral health, or social services experience is required.
  • Minimum of two years of experience working directly with youth and young adults, those experiencing homelessness preferred.

Responsibilities

  • Ensure care coordination and housing-focused services prioritize rapid placement in stable housing.
  • Provide individualized case management services, life skills education, and linkage to resources.
  • Foster life skill development and independence through personalized support.
  • Develop and oversee individualized treatment plans with YYAs and their service teams.
  • Engage, mentor, and meet with YYAs regularly, promoting trusting relationships.
  • Conduct home visits at least once per month to assess progress.
  • Assist YYAs in developing schedules, accompany to court dates and SSI hearings.
  • Assist YYAs in developing an exit plan and provide aftercare services.

Position Summary

Provide individualized, therapeutic-based support to youth and young adults (YYAs) experiencing homelessness. Support YYAs throughout the full continuum of care while engaged in Daybreak’s programs. Help YYA’s transition from shelter to securing safe and stable housing while connecting them to programming and resources that promote long-term stability and overall well-being. Work closely with clients to create individualized service plans that emphasize self‑sufficiency, community integration, and holistic care, tailored to each client’s unique needs and strengths.

The Care Coordinator builds trusting relationships with YYAs and collaborates with service providers to connect clients to housing, employment, healthcare, and personal development opportunities. The Care Coordinator is a strong communicator with heightened cultural competence, persistence, and a passion for helping young people move from homelessness to independence and stability.

Duties and Responsibilities
Case Management and Client Support
  • Ensure all care coordination and housing‑focused services prioritize rapid placement in stable housing without preconditions while offering flexible, youth‑centered support that promotes independence and resilience for long‑term success.
  • Provide individualized case management services, life skills education, and linkage to educational, employment, financial, medical, housing, and other social resources tailored to the YYA’s treatment plan goals.
  • Foster life skill development, social skills enhancement, independence and stability through personalized support, empowering YYAs to overcome challenges and achieve their goals.
  • Develop and oversee the implementation of individualized treatment plans with YYAs and their service teams, identifying strategies or treatment options to ensure progress toward YYA goals.
  • Engage, mentor, and meet with YYAs regularly as determined by program standards, promoting trusting relationships while delivering care coordination services throughout the full continuum of care including YYA choice and trauma‑informed approaches.
  • Conduct home visits as needed, ensuring at least one visit per month to the YYA’s apartment to assess progress and identify areas for growth as they work toward independence.
  • Assist YYAs in developing schedules to track activities and appointments, ensuring consistency and follow‑through. Accompany YYAs to court dates, SSI hearings, and other official appointments, advocating for their needs when appropriate.
  • Assist YYAs in developing an exit plan and provide aftercare services to ensure long‑term housing stability.
Collaboration and Coordination
  • Act as a central point of communication among team members and external community providers to promote a collaborative, youth‑centered approach. Regularly evaluate progress and adjust strategies as needed to ensure YYA goals are achieved.
  • Collaborate with Daybreak providers and external community partners to help YYAs progress toward their individualized treatment plan goals.
  • Coordinate YYA’s activities with the service team, ensuring their needs are met and barriers to success are identified and addressed promptly. Provide guidance and support to ensure responsibilities are completed successfully.
Documentation and Compliance
  • Maintain monthly productivity according to standards set by Senior Management by providing individual and group services, as appropriate.
  • Complete UIR (Unusual Incident Report) and other forms that may be required in CareLogic or for PQI as assigned and as required.
  • Maintain accurate client records in compliance with agency procedures and funder requirements, ensuring all documentation is completed within 72 hours as per agency standards.
Qualifications
  • Associate’s degree in social work, human services, psychology, criminal justice or a related field or 2 years of college from an accredited four‑year college or university is required.
  • A minimum of two (2) years of program management, case management, behavioral health, or social services experience is required.
  • A minimum of two (2) years of experience working directly with youth and young adults is required, those experiencing homelessness or housing instability preferred.
