The Care Coordinator is an integral member of the interdisciplinary team, with expertise in assessment, discharge planning, and psychosocial environmental aspects of patient and family dysfunction. Demonstrates competency with interventions and clinical judgment based on the population and age-specific needs. Utilizes clinical expertise to provide group, family, and individual therapy through various modalities to aid in successful transition readiness.
