Job summary

Broadlawns is seeking a Care Coordinator for the Crisis Hall to join our Full Time team. You will conduct discharge planning, psychosocial assessments, and counseling for clients in crisis, coordinating with a multidisciplinary team.

The role emphasizes collaboration, de-escalation skills, and precise documentation. A Bachelor's in human services and a valid driver's license are required; Iowa-specific credentials may apply.

Qualifications

• Bachelor's degree in human services or related field.
• 2+ years working with individuals with chronic mental illness.
• Ability to work cooperatively with a multi-disciplinary team.
• Excellent written and verbal communication skills.
• Ability to verbally and physically de-escalate a client using trained programs.
• Ability to complete electronic charting and documentation.
• Valid Iowa Driver's License with ability to obtain Iowa Chauffer's license within one month of hire with CSC.
• For Mobile Crisis, must have valid Iowa Driver's License without restrictions.

The individual works in collaboration with other department area staff to provide a full array of Behavioral Health support. **PERFORMANCE STANDARDS**1.
