WHAT YOU'LL DO

As a Care Coordinator, Court Diversion, you'll work closely with clients, families, treatment providers, community agencies, and members of the legal system to coordinate services and help individuals successfully navigate their care.

Your responsibilities may include:

  • Complete comprehensive assessments to identify each client's individual strengths, challenges, and service needs.
  • Work collaboratively with clients to develop a Plan of Care with meaningful goals and objectives that support their individual needs and recovery.
  • Connect clients and their families with treatment, support services, and community resources identified in their Plan of Care.
  • Maintain communication with providers, agencies, and other members of the client's support network to monitor progress and ensure services continue to meet their needs.
  • Advocate on behalf of clients with community agencies, legal entities, court representatives, and Meridian programs.
  • Conduct face-to-face visits to monitor client progress and provide support in a variety of settings, including clients' homes, courtrooms, jails, residential programs, and inpatient psychiatric facilities.
  • Collaborate with court personnel, jail staff, public defenders, State Attorney representatives, treatment providers, and other community partners.
  • Maintain accurate and timely client records, progress notes, and required documentation.
  • Document services and activities provided on behalf of clients in accordance with program and organizational requirements.
  • Research, develop, and maintain knowledge of community resources available to support clients and their families.
  • Help clients navigate barriers to services while promoting engagement, independence, stability, and recovery.
  • Perform additional duties as needed to support the goals of the Court Diversion program and the individuals we serve.
WHAT YOU'LL BRING
Education & Experience
  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Minimum of 1 year of experience working with mental health populations.
  • Forensic or criminal justice experience is preferred.
  • Experience working with individuals who may have complex behavioral health, social, or community-based needs is valuable in this role.
YOUR CARE COORDINATION TOOLKIT

You’ll be successful in this position if you’re compassionate, organized, resourceful, and comfortable working with people from a variety of backgrounds and circumstances.

You’ll bring:

  • Compassion & Empathy: The ability to meet individuals where they are and provide support without judgment.
  • Client Advocacy: A genuine desire to help individuals identify resources, overcome barriers, and access the services they need.
  • Relationship Building: The ability to establish positive working relationships with clients, families, treatment providers, community partners, and representatives of the legal system.
  • Communication Skills: Strong verbal and written communication skills with the ability to communicate effectively across multiple settings.

  • A commitment to treating every individual with dignity, respect, and compassion.
TRAVEL & COMMUNITY-BASED WORK

This role goes beyond the traditional office setting. Supporting our clients may mean meeting them wherever services and advocacy are needed.

  • A valid Florida driver's license and acceptable driving record are required.

A CAREER WITH PURPOSE

At Meridian, care coordination is about more than connecting someone to a service. It's about helping people see that their current circumstances don't have to define their future.

As part of our Court Diversion program, you'll have the opportunity to advocate for individuals, help them navigate complex systems, connect them with meaningful resources, and support their journey toward recovery and stability.

Some days you'll be coordinating services. Other days you'll be advocating for a client in court, connecting with a community partner, or simply helping someone take the next step forward. Whatever the day brings, you'll know your work has the potential to make a real difference.

If you're compassionate, resourceful, and passionate about behavioral health and helping others build a better future, we'd love to hear from you.
