Job summary

Meridian Behavioral Healthcare, Inc. in Florida is hiring a Care Coordinator to help adults with mental health, substance use, and co-occurring disorders who are involved in or at risk of entering the criminal justice system.

Are you passionate about helping individuals overcome barriers and achieve recovery? Meridian Healthcare is seeking a compassionate and motivated Care Coordinator to support adults with mental health, substance use, and co-occurring disorders who are involved in, or at risk of entering, the criminal justice system.

In this rewarding role, you will work directly with individuals in the community, helping them access treatment, navigate resources, develop recovery goals, and successfully transition toward healthier, more stable lives.

What You'll Do
  • Conduct comprehensive assessments to identify client needs and strengths.
  • Develop and coordinate individualized service and recovery plans.
  • Connect clients with community resources, healthcare services, housing, and supports.
  • Monitor client progress and provide ongoing support and advocacy.
  • Collaborate with courts, jails, treatment providers, and community partners.
  • Maintain timely and accurate documentation in accordance with program standards.
What We're Looking For
Minimum Qualifications
  • Bachelor's degree in Psychology, Social Work, Healthcare, or a related field.
  • Experience working with jail diversion programs, courts, criminal justice populations, or related community services preferred.
  • Valid Florida Driver's License with a clean driving record that meets company insurance requirements.
