Job summary

The MetroHealth System in Brooklyn Heights is hiring a registered nurse to join the Cancer Care case management team. You will collaborate with the care team to coordinate patient services, perform clinical review, and plan care across clinic and home visits.

Required: BSN, Ohio RN license, 3–5 years in case management; strong communication and analytical skills. Oncologic certification (OCN/BMTCN) or eligibility within 12 months preferred; Master’s degree and EPIC experience are a plus.

Qualifications

• Bachelor of Science in Nursing and Ohio RN licensure.
• 3–5 years of case management or managed care experience.
• Ability to work independently and in a multidisciplinary team; strong communication, delegation, problem-solving, and analytic skills.
• Cancer Care: minimum 5 years of clinical practice; oncology certification (OCN or BMTCN) or obtain within 12 months of hire.
• Preferred: Master's degree, case management certification, EPIC familiarity, and case management software knowledge.

Summary:

Collaborates with the care team in the provision of patient care. Performs a clinical review of selected patients to determine service needs. Coordinates the care of selected patients. Integrates and coordinates access and utilization management, proactive patient panel management, care facilitation, and treatment planning functions. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications:

Required:

• Bachelor of Science in Nursing.
• Valid Ohio RN licensure.
• Three to Five years of case management or managed care experience.
• Ability to work independently and as a member of a multidisciplinary team Demonstrates excellent interpersonal, verbal/written communication and delegation and problem-solving skills Demonstrates strong analytical, data management and computer skills Demonstrates strong leadership, organizational and time management skills Reliable transportation to make clinic or home visits to patients as needed Additional requirements for Cancer Care: Minimum 5 years of clinical practice experience.
• Oncology Certified Nurse (OCN) or Blood & Marrow Transplant Certified Nurse (BMTCN), or obtains within 12 months of hire.

Preferred:

• Master's Degree.
• Case Management Certification.
• Experience with case management, access management, home health care, psychiatric case management, discharge planning or related experience.
• Knowledge of EPIC functionality.
• Knowledge of case management software.

Physical Demands:

• May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating.
