Job summary

CenterPointe is seeking a full-time Care Coordinator in Lincoln, NE. You will provide targeted support to individuals and families, helping navigate community resources and coordinating care during transitions.

Qualifications

• At least 21 years of age and valid driver's license.

THE JOB

As a Care Coordinator, you will provide targeted support to individuals receiving services, and assisting individuals and families to navigate community resources. You will follow-up with individuals care during transitions of care and to increase engagement in services. Ensure that coordinated systems of care are responsive to the assessed needs of individuals we service in a timely and comprehensive manner.The position is full-time, Monday through Friday.

Qualifications

WHAT YOU HAVE

Required:

• High school diploma or equivalent
• Two or more years of experience working in Behavioral Health
• Knowledge of co-occurring disorders and related needs
• At least 21 years of age
• Valid driver's license with acceptable DMV record (last 7 years)
