Job summary

RemoteHunter is seeking a Care Coordinator to manage the care of members with complex behavioral health needs in a United States setting. You will conduct assessments, develop care plans, and coordinate with interdisciplinary teams to ensure quality, cost-effective care.

You will advocate for members, monitor plans, educate providers and families, analyze data for service improvements, and generate reports that align with care coordination goals.

Qualifications

• 3-5 years of experience in Social Work, Nursing, Healthcare, or related fields.
• Experience in utilization management, quality assurance, community health, or long-term care.
• Strong written and verbal communication skills.

• Knowledge of referral processes to community and private/public resources.
• Ability to analyze data and make complex decisions beyond existing protocols.
• Strong verbal and written communication skills.

5. Pay Range and Compensation Package:

• Salary range: $50,225 to $75,335 annually, adjusted based on location, skills, and experience.
• Eligibility for short-term incentives.
