Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources. We serve southeastern and Central Ohio with a comprehensive array of behavioral health and other services, working with local partners to promote healthy people and strong communities. Our services are intended to be collaborative and personalized for the individual.
OhioRISE is a Medicaid managed care program that provides specialized services for youth ages 0-20 with complex behavioral health and/or substance use needs.
The OhioRISE Care Coordinator is responsible for creating, planning, and organizing a team as defined by the family. This position facilitates child & family team meetings that support youth at home, in school, and in their communities with the goal of keeping families together and reducing hospitalizations. The team may include partnering with social service providers, community partners, and the families support systems. The care coordinators role includes leading the team in identifying needs, goals, and action steps to meet the families goals. The care coordinator ensures the youth and families have a voice and choice in all coordinated care and services provided.
The pay range for this position is $20.19 - $25.03 per hour based on experience, education, and/or licensure.
