Essential Functions

• Develop individualized care plans with prioritized goals considering patient and caregiver preferences.
• Assess patients' health status through outbound calls.
• Identify gaps or barriers in treatment plans and coordinate care, including referrals.
• Ensure smooth discharge processes with necessary services and medication reconciliation.
• Triages symptom-based calls per protocols for various patients.
• Enroll patients in home monitoring programs and manage alerts.
• Provide general health guidance from approved sources, assessing caller needs.
• Document workflows in EPIC system for tracking and quality purposes.
• Educate patients on self-management, chronic, and acute disease processes.
• Partner with providers to meet quality and cost management goals.
• Monitor enrolled members throughout the care continuum.
• Engage with leadership and physician groups to discuss clinical cases.
• Participate in case conferences to develop tailored care strategies.

Job Qualifications

• LA CNA Certification required.
• Adult First Aid & CPR Certification required.
• Memory care experience preferred.
• Experience working with elderly preferred.

Work Shift

Variable hours, United States based.
