Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC) is seeking an experienced and compassionate Care Coordination Nurse Manager to lead our care coordination and case management programs. Position Summary

The Care Coordination Nurse Manager oversees essential functions required by the HRSA Health Center Program, including referral coordination, post‑hospitalization tracking, and value‑based care initiatives. You will guide a dedicated team, strengthen clinical workflows, and ensure patients receive seamless, coordinated care across the healthcare continuum.

Key Responsibilities

Lead & Support Your Team

  • Provide daily leadership and oversight for the Patient Care Coordinator (PCC) team.
  • Ensure adequate coverage, efficient workflows, and adherence to organizational policies.
  • Foster a positive, collaborative, and mission‑aligned work environment.

Enhance Clinical & Operational Performance

  • Analyze and refine clinical and EHR workflows to improve patient outcomes.
  • Collaborate with the Medical Director and Clinical Support teams to implement standardized processes.
  • Track and report performance metrics, including monthly dashboards and UDS measures, to senior leadership.

Champion Patient-Centered Care

  • Identify unmet patient needs using evidence‑based guidelines.
  • Ensure the care coordination team engages patients in shared decision‑making.
  • Support continuity of care by tracking referrals and monitoring hospital and emergency room utilization.

Strengthen Community Partnerships

  • Build relationships with hospitals, specialists, and community organizations.
  • Connect patients with appropriate community resources to improve health outcomes and reduce avoidable utilization.
  • Support seamless transitions of care for at‑risk populations.

Advance Quality & Compliance

  • Use the EHR to manage population‑level data and generate meaningful reports.
  • Participate in committees and develop data‑driven reports and corrective action plans.
  • Support preparation for HRSA Operational Site Visits (OSV) and clinical audits.
Qualifications
  • Registered Nurse (RN) required; BSN preferred
  • Experience in population health or care coordination
  • 3–5+ years of clinical nursing experience (ambulatory or community health preferred)
  • Strong computer proficiency, including advanced Excel and data analysis
  • Excellent written and verbal communication skills
  • Experience with statistical analysis, report interpretation, and quality improvement
  • Knowledge of community health and social service networks
  • Strong interpersonal skills and the ability to work effectively across diverse teams
  • Valid NJ driver’s license and reliable transportation

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $80,000.00 to USD $120,000,00. When determining an offer, VNACJ CHC considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add‑on incentives, special clinical compensation, retirement plans, and all other benefits.
