Job summary

Humana is seeking a Registered Nurse to conduct comprehensive face-to-face assessments for admitted members at UK King's Daughters Medical Center, coordinate discharge planning, and refer to Humana programs as appropriate. Requirements include Kentucky RN licensure, 3+ years acute care experience, and EMR proficiency.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Must live within 30–60 minutes of UK King's Daughters Medical Center.
• Minimum 3 years of acute care nursing experience.
• Experience with discharge planning or care coordination in hospital/health plan settings.
• Experience with Microsoft Office (Word/Outlook/Excel).
• Experience with electronic medical records.
• Valid driver’s license and dependable automobile.
• Proof of personal vehicle liability insurance.
• TB screening participation.
• ACHA Level II background clearance with fingerprints when required.
• Other facility credentialing/screening as required.
• Private workspace for remote documentation.
• Home internet with 25 Mbps down / 10 Mbps up.
• BSN preferred.
• Care management/health promotion/coaching experience preferred.
• Utilization Management experience preferred.
• Health plan experience preferred.
• Bilingual English/Spanish preferred.
• Care Management certification or related specialty preferred.
• HEDIS and Stars measures knowledge preferred.

Responsibilities

• Evaluate members’ physical, psychosocial, and health needs and refer them to Humana programs as appropriate.
• Support and educate members and families on discharge planning from admission through post-discharge.
• Facilitate follow-up visits with primary care providers.
• Identify gaps in care, safety concerns, and barriers to discharge.
• Develop and implement strategies to address care and discharge issues.
• Collaborate with attending providers, facility case managers, and healthcare professionals.
• Provide education on diagnoses, medications, and preventive measures.
• Complete required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
• Coordinate care onsite at UK King's Daughters Medical Center and remotely document as permitted.

• Dependable automobile
• Proof of personal vehicle liability insurance
• Participation in TB screening
• ACHA Level II background clearance, including fingerprinting, as required
• Other credentialing and screening as required by the assigned facility
• Private workspace for remote documentation
• Home internet with at least 25 Mbps download and 10 Mbps upload speeds
• Bachelor's degree in Nursing (BSN) preferred
• Care management, health promotion, or coaching experience preferred
• Utilization Management experience preferred
• Health plan experience preferred
• Bilingual English/Spanish preferred
• Certification in Care Management or related specialty preferred
• Knowledge of HEDIS and Stars measures preferred
