Qualifications

• Minimum two (2) years of experience in a health-related setting or Community Based Organization.
• Licensure and/or Certification as required by discipline.

Responsibilities

• Interject objectivity and work collaboratively on healthcare plans.
• Assist clients in self-directed care decisions.
• Develop comprehensive healthcare plans addressing clients' needs.
• Make independent assessments of clients' progress.

Knowledge of funding resources and clinical standards.

Education

Master's in Social Work or R.N. with Master's in Health Related Field.

Completion of NYS AIDS Counseling Training.
