Qualifications

• Bachelor's degree is required in criminal justice, social work, psychology, mental health, human services, or a related field.
• Master's degree preferred for advancement and scope of practice.
• CASAC license is required or strongly preferred.
• Knowledge of medication-assisted treatment practices is required.
• Experience with individuals involved in the criminal justice system is preferred.

Responsibilities

• Assess patients' discharge planning needs and identify barriers to successful transition.
• Develop individualized discharge plans with patients and interdisciplinary team.
• Coordinate discharge planning activities from admission through discharge to ensure continuity of care.
• Coordinate referrals to medical, behavioral health, social services, housing, and community programs.
• Schedule follow-up medical, psychiatric, counseling, and primary care appointments.
• Coordinate medication needs and ensure prescriptions are addressed before discharge.
• Communicate with hospitals, rehab facilities, and outside providers.
• Educate patients about discharge plans, follow-ups, medications, and resources.
