Job summary

Qualifications

• Performs comprehensive nursing assessment/reassessment.
• Accurately documents findings in the patient record.
• Administers and documents medications per policy.
• Provides patient and family education.
• Collaborates with health team for daily plan of care.
• Incorporates infection control practices in daily care.
• Performs ongoing evaluation of care effectiveness and adjusts plan of care.

Responsibilities

• Collaborates with health team for daily plan of care and accurate handoff reports.
• Provides and documents nursing care and changes in patient condition.
• Administers medications and monitors IV fluids as per policy.
• Completes referrals and requests consultations as needed.
• Delivers patient/family education and discharge planning.
• Ensures physician orders are clarified and implemented in a timely manner.
