Job summary

Alteas Health seeks a Nurse Practitioner or Physician Assistant to deliver specialized pulmonology care across Skilled Nursing, Long-Term Care, and Post-Acute settings in Tinley Park and nearby communities. The role emphasizes clinical autonomy, physician collaboration, and daily travel within the territory.

The position requires NP/PA licensure, national certification, BLS, and at least 6 months of clinical experience, plus travel ability and EMR proficiency.

Qualifications

• Current Nurse Practitioner or Physician Assistant license in the applicable state(s).
• National certification appropriate to licensure.
• Current BLS certification.
• Minimum six (6) months of clinical experience.
• Valid driver’s license with reliable transportation.
• Ability to travel daily within assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience with Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based practice.
• Order, interpret, and communicate lab studies, imaging, and other tests.
• Prescribe medications within scope of practice.
• Educate patients and families about diagnoses and care plans.
• Coordinate care with physicians, nursing staff, and other teams.
• Maintain accurate clinical documentation.
• Round at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build relationships with facility leadership and teams.
• Represent Alteas Health with professionalism and clinical excellence.
