The advanced practice provider (Nurse Practitioner/Physician Assistant) within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care in the Cardio-Oncology Program, primarily based out of Center City, PA, but may include regional centers such as Asplundh in Abington, PA.
The Cardio-Oncology Program is a unique multidisciplinary initiative working closely with colleagues from SKCCC. Patients include individuals being treated for any form of malignancy (hematologic, solid tumor, etc.) or candidates for SCT/BMT, who may have underlying cardiovascular disease or develop cardiovascular complications secondary to oncologic treatments.
Program Highlights:
- Busy, unique program in Cardio-Oncology in Center City at TJUH, collaborating with SKCCC.
- Patient population comprises adults with cancer referred for cardiovascular assessment/management/monitoring before, during, and after treatment, including survivorship care.
- The candidate will perform complete history and physicals, document findings in medical records, and prescribe appropriate cardiac treatments, including testing and medications.
- The candidate will help develop protocols for cardiotoxicity management and survivorship initiatives, and may engage in community outreach if desired.
- Inpatient consultations will be performed as the program develops an inpatient consultative service.
- Participation in clinics at Honickman during normal work hours is expected.
- Telemedicine will be incorporated into daily practice as appropriate.
- Expected to develop skills in interpreting laboratories and ECGs.
- No call or holiday responsibilities.
Qualifications:
- Nurse Practitioner (CRNP/ACNP) or Physician Assistant certification required.
- Certification such as PALS preferred.
Assessment & Management Responsibilities:
- Perform comprehensive history and physicals, document in medical records.
- Order and collect data using assessment techniques and diagnostic procedures.
- Serve as first responder for nursing and house staff during patient status changes, including procedural and resuscitative interventions.
- Assess risks related to medication side effects, immobility, nutrition, fluid/electrolyte balance, immunocompetence, invasive procedures.
Diagnosis:
- Collaborate with the healthcare team to diagnose acute and chronic conditions.
- Manage diagnostic tests, interpret results, and formulate differential diagnoses.
- Diagnose complications and order interventions.
Plan of Care:
- Identify expected outcomes, develop, and document individualized plans of care.
- Use evidence-based guidelines and protocols.
- Implement and modify plans; prescribe diagnostic and therapeutic interventions.
- Perform advanced procedures as privileges and competencies allow.
Documentation:
- Document patient progress via progress notes, transfer/discharge summaries, and clinic notes, ensuring timeliness and compliance.
Communication & Collaboration:
- Maintain ongoing communication with the healthcare team, patients, and families to promote continuity of care.
Professional Practice:
- Demonstrate professional behaviors, including mentoring, education, and research participation.
- Engage in continuing education and professional organizations.
