Job Summary

We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.

Responsibilities

• Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests.
• Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions.
• Administer medications, monitor patient responses, and adjust treatment plans as necessary.
• Conduct cardiac stress tests, electrocardiograms (EKG), and other diagnostic procedures.
• Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence.
• Provide counseling and emotional support to patients and their families.
• Maintain knowledge of CMS Regulations and NCQA HEDIS Guidelines.
• Document accurate medical records to ensure compliance with OSHA and Government regulations and perform duties in accordance with site quality and safety systems.
• Collaborate with the interdisciplinary team.
• Analyze and organize the care of critically complex patients.
• Perform travel as required to the following cities: Torrington, Glastonbury, Hamden, and Wallingford.

Requirements

• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License.
• CPR BCLS/BLS certified.
• Must hold a Master's Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• Preferred experience in cardiology (training provided).
• Preferred experience in documenting EMR (Sigma, Visual, PCC, etc.) (training provided).
• Strong organizational and time management skills.
• Ability to lift a minimum of 20 pounds and stand, walk, or work on your feet up to 6-8 hours per day.
