Job summary

Infinite Medical P.C. is seeking a Cardiology Nurse Practitioner or Physician Assistant to join our Stamford team. You will evaluate patients with heart conditions, work with cardiologists on treatment plans, and ensure high-quality care in a long-term care setting.

The role requires an NP/PA license, an active DEA, CPR/BLS, and a Master’s in Nursing, with at least 6 months in a SNF. The position is full-time with generous compensation and benefits.

Qualifications

  • Active and unrestricted NP/PA/Physician license.
  • DEA license is required.
  • Master's degree in nursing.
  • Completed a NP program from an accredited school.
  • Minimum 6 months of long-term care or SNF experience.
  • Preferred cardiology experience or training.
  • Strong organizational and time management skills.
  • Ability to stand/walk for extended periods and lift up to 20 lbs.

Responsibilities

  • Perform comprehensive patient assessments and medical histories.
  • Collaborate with cardiologists to develop treatment plans.
  • Administer medications and monitor responses.
  • Conduct cardiac stress tests and EKGs.
  • Educate patients and families on cardiovascular health and adherence.
  • Document medical records in EMR according to regulations.
  • Coordinate care with interdisciplinary team.
  • Manage care for critically complex patients.

Job Summary

We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions. Responsibilities
  • Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests
  • Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions
  • Administer medications, monitor patient responses, and adjust treatment plans as necessary
  • Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures
  • Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence
  • Provide counseling and emotional support to patients and their families
  • Knowledge of CMS Regulations and NCQA HEDIS Guidelines
  • Document accurate medical records to ensure compliance with OSHA and Government regulations and perform duties in accordance with site quality and safety systems
  • Collaborate with the interdisciplinary team
  • Analyze and organize the care of critically complex patients
Requirements
  • Minimum 6 months of experience in long-term care or a skilled nursing facility
  • Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
  • DEA License
  • CPR BCLS/BLS certified
  • Must hold a Master's Degree in Nursing
  • Completed a degree from an accredited Nurse Practitioner program
  • Preferred experience in cardiology: training provided
  • Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
  • Must have great organizational and time management skills.
  • Prolonged standing/walking. Occasional lifting up to 20 lbs. Carrying medical equipment. Lifting/moving patients.
