Cardiology Nurse Practitioner/ Physician Assistant

Location: Hamden and Danbury

Schedule: Part-Time (1 day per week)

Salary: $60,000 - $90,000

Job Summary

We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.

Responsibilities

  • Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests
  • Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions
  • Administer medications, monitor patient responses, and adjust treatment plans as necessary
  • Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures
  • Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence
  • Provide counseling and emotional support to patients and their families
  • Assist with cardiac catheterizations, angioplasties, and other invasive procedures under the supervision of a cardiologist.
  • Medication Management: NPs/PAs ensure residents receive the appropriate medications, monitor for side effects, and adjust dosages as needed. Chronic Disease Management: They provide ongoing care for residents with chronic conditions such as diabetes, heart disease, and dementia, helping to manage symptoms and improve quality of life.
  • Wound Care: NPs/PAs are skilled in wound assessment and treatment, promoting healing and preventing complications.
  • Fall Prevention: They identify residents at risk for falls and implement strategies to reduce fall incidents.
  • Knowledge of CMS Regulations and NCQA HEDIS Guidelines
  • Document accurate medical records to ensure compliance with OSHA and
  • Government regulations and perform duties in accordance with site quality and
  • safety systems
  • Collaborate with the interdisciplinary team
  • Analyze and organize the care of critically complex patients

Requirements

  • Minimum 6 months of experience in long-term care or a skilled nursing facility
  • Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
  • DEA License
  • CPR BCLS/BLS certified
  • Must hold a Master's Degree in Nursing
  • Completed a degree from an accredited Nurse Practitioner program
  • Preferred experience in cardiology: training provided
  • Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
  • Must have great organizational and time management skills.
