Job summary

Qualifications

  • Current license from Commonwealth of Kentucky as Nurse Practitioner.
  • Completion of an accredited Nurse Practitioner program.
  • Current BLS, ACLS, and PALS certifications.
  • Experience: minimum of one year preferred.

Responsibilities

  • Obtain a relevant and complete health and medical history and perform appropriate physical examination.
  • Perform or order preventive and/or diagnostic tests or procedures based on age and history.
  • Identify and discuss pathophysiology of disease processes.
  • Formulate appropriate diagnosis based on history, exam and tests.
  • Order diagnostic tests, review results and report to patient.
  • Prescribe therapies according to privileges.
  • Manage general medical conditions and understand related medications and treatments.
  • Identify, respect and address patients' differences and preferences.
  • Plan and provide care involving patient and family in planning.
  • Provide patient/family education using active listening.
  • Evaluate effectiveness of care plan and intervene as needed to planned outcomes.
  • Initiate appropriate referrals and document accurately.

Job Location: CADIZ, KY 42211Position Type: PRNPatient Care -

  • Obtain a relevant and complete health and medical history and performs appropriate physical examination.
  • Perform or order appropriate preventive and/or diagnostic tests or procedures based on the patient’s age and history.
  • Ability to identify and discuss pathophysiology of disease processes
  • Formulate appropriate diagnosis based on the history, physical examination and diagnostic test results
  • Order diagnostic tests. Review results and report to patient.
  • Prescribe appropriate pharmacologic and non-pharmacologic therapies, according to privileges.
  • Manage general medical conditions to include understanding the indications, contraindications, side effects, interactions, and adverse reactions of pharmacologic agents and other relevant treatment modalities.
  • Identify, respect, and show compassion regarding patients’ differences, values, preferences and expressed needs during interaction.
  • Plan and provide care, within the framework of policies, procedures, privileges and available resources. Involves the patient and family in planning care.
  • Provide patient/family education using active listening.
  • Evaluate effectiveness of plan of care and intervenes by response to planned outcome
  • Initiate appropriate referrals
  • Document in an accurate and timely manner.
  • Advocate disease prevention, wellness and promotion of healthy lifestyle choices.
Other Duties -

Report to Practice Manager or Administrator on-call significant incidents such as:

  • Unusual patient or family problems or concerns
  • Physician concerns
  • Major damage or utility outages
  • Security problems
  • Safety issues
  • Matters of legal importance
  • Calls received from any regulatory body
  • Report incidents in Quality Calendar

Participate in Medical Staff functions and activities to include review of policy and procedure.

Other duties related to hospital business and patients as assigned (i.e. participate in community activities to promote clinic services and provide in patient education).

QualificationsQUALIFICATIONS:
  • Licensure / Certification / Registration: Current license from Commonwealth of Kentucky as Nurse Practitioner. Evidence of continuing education credits as required by license.
  • Education: (be specific): Completion of an accredited Nurse Practitioner program. Current BLS, ACLS, and PALS certifications.
  • Experience: A minimum of one year experience is preferred. Supervisory experience preferred, but not required.
