Job summary

The University of Kansas Health System is seeking an Ambulatory Clinic RN Care Coordinator (RN) for the Cardiothoracic Surgery Clinic. This on-site role provides clinical nursing care, patient education, and care coordination within a busy CTS ambulatory setting.

You will support physicians, manage patient flow, perform triage, and ensure accurate documentation in the EMR. A BSN and 2+ years RN experience are required; BLS certification is expected.

Qualifications

• High School Graduate
• Bachelor Degree in Nursing
• 2 or more years of RN experience.

Responsibilities

• Manage patient flow, triage and direct patient care.
• Liaise between front desk, nursing staff, physicians and clinic manager.
• Manage patient outcomes and resource utilization with provider goals.
• Provide nursing support and coordination to optimize outcomes and education.
• Support patient self-management of disease.
• Coordinate continuity of care after hospital admission, discharge, and ER visits.
• Manage high-risk patient care and maintain a patient registry.
• Educate patients and family about health care needs and safety.
• Document assessments and plans in EMR.
• Observe and report patient condition and drug/treatment responses to physicians.
